Job summary

New Vanderbilt Rehabilitation and Care Center is seeking a dedicated Social Worker to join our skilled nursing and rehabilitation team. We provide sub-acute medical care, rehabilitation services, and 24-hour skilled nursing care in a compassionate, respectful environment.

Responsibilities include collaborating with disciplines, discharge planning, and educating residents and families on services and equipment needs. We value a supportive work environment and commitment to resident well-being.

Qualifications

• Year of active patient/resident contact in a health care setting.
• At least 2 years of experience in a long-term care setting.
• Experience with MDS, discharge planning and electronic medical records a plus.

Responsibilities

• Work collaboratively with members of all disciplines to ensure high quality care for residents and families.
• Implement, analyze, and communicate comprehensive risks to ensure appropriate care.
• Meet with residents and families to discuss progress, goals, obstacles, and expectations.
• Educate residents and families on continued services and possible equipment needs.
• Develop and maintain a quality relationship with the medical community and related facilities.
• Complete BIMs, Mood, Psychosocial, Behavior, and Discharge observations timely.
• Interpret social, psychological, and emotional needs of residents.
• Manage complaints and grievances per policy and procedure.
• Ensure charted progress notes are informative and descriptive.
