Qualifications

• Minimum one year of experience in healthcare, rehabilitation, or long-term care preferred.
• Experience with discharge planning and interdisciplinary team collaboration.

Responsibilities

• Complete psychosocial assessments and social service documentation.
• Participate in interdisciplinary care planning.
• Provide counseling, support, and education to residents and families.
• Coordinate discharge planning and community services.
• Maintain accurate documentation within the Electronic Medical Record (EMR).
