Qualifications

• Master's degree from a Council on Social Work accreditation.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assist patients and care team to understand social, emotional, environmental factors related to health.
• Contribute to the development of a comprehensive Plan of Care for patients.
• Instruct health care team on community resources available to patients.
