Qualifications

Responsibilities

• Interview clients to assess medical, psychological, emotional, and social information; visit on-site.

• Maintain medical records and case documentation and monitor progress with follow-ups.
• Stay compliant with policies, procedures, and legal requirements; participate in quality reviews.
• Engage in interdisciplinary conferences and provide consultation for clinical staff.
