Job summary

Nurse On Call, a part of LHC Group and a nationwide post-acute care partner within UnitedHealth Group, seeks a Social Worker - Per Diem. You will deliver medical social services under physician direction with an interdisciplinary team to address social and emotional factors affecting health.

Required: MSW from a CSWE-accredited program, valid driver's license and access to reliable transportation; preferred: 1+ year healthcare social work, CPR certification within 90 days, and licensed Social

Qualifications

• Master's Degree from a CSWE-accredited School of Social Work.
• Valid driver's license, vehicle insurance, and access to dependable transportation.
• 1+ years of social work experience in a health care setting (preferred).
• Current CPR certification or ability to complete within 90 days of hire (preferred).
• Licensed Social Worker in the state of residence (preferred).

Responsibilities

• Provide medical social services under the direction of a physician and Interdisciplinary Group.
• Assist in understanding significant social and emotional factors related to the patient's health status and coping.
• Support patients and families in navigating care transitions and community resources.
