Job summary

AtlantiCare is seeking a Social Worker BSW for the Galloway pool to join an interdisciplinary team providing non-clinical social work services within the care plan. The role emphasizes assessment, counseling and coordination of care, with per diem, part-time scheduling and collaboration with physicians to support patient outcomes.

Requirements include a Bachelor's degree in social work and CSW certification, plus at least one year of healthcare-related experience.

Qualifications

• Bachelor's degree from an accredited school of social work.
• CSW Social Worker Certificate required.
• Minimum 1 year experience, preferably within healthcare.
• Proficiency in service applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required.

Responsibilities

• Oversee coordination of the interdisciplinary plan of care and ensure alignment with medical treatment plans.
• Provide assessment, counseling and guidance to patients and families as part of the social work process.
• Collaborate with the healthcare team to problem-solve and connect patients to resources.
