Qualifications

• Bachelor's degree in social work required.
• CSW Social Worker Certificate required.
• Minimum 1 year experience, preferably within healthcare.
• Proficiency in service Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

Responsibilities

• Coordinate interdisciplinary plan of care and ensure alignment with medical treatment plan.
• Provide non-clinical social work interventions and guidance to individuals and families.
• Engage in assessment, counseling, consultation, and planning as part of the patient care process.
