AtlantiCare is seeking a Social Worker to join the BSW Pool in Atlantic City. The role involves coordinating care with an interdisciplinary team, conducting assessments, and guiding patients and families through the social care plan. Experience in healthcare and CSW certification are required.

Qualifications

• Bachelor's degree from an accredited School of Social Work is required.
• CSW Social Worker Certificate required.
• Minimum 1 year experience, preferably in healthcare; proficiency with service applications.

Responsibilities

• Assess patient needs and coordinate the interdisciplinary plan of care.
• Provide counseling and guidance to individuals, families or groups.
• Document actions and refer to appropriate resources as part of care planning.
• Support performance improvement initiatives within the patient care team.
