Responsibilities

• Review evaluations, care objectives, and plans with the team to coordinate services.
• Advocate for needed services and entitlements for patients.
• Assist in discharge planning and coordinate aftercare with the clinical team.
• Communicate effectively with physicians, therapists, patients, and families.
• Coordinate post-discharge resources for homecare or alternate care as needed.
• Ensure compliance with state, federal regulations and accreditation standards.
