Job summary

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for a per diem home health role serving Montgomery County and Frederick County, MD. You will assess and treat clients with speech, language, and swallowing difficulties in their homes, and collaborate with a multidisciplinary team to plan and adjust care.

Responsibilities include evaluating, planning, implementing, and documenting therapy, plus educating patients and families.

Qualifications

• A current license as a Speech Language Pathologist in MD.
• CCC-SLP or equivalent certification; eligible for examination.
• Experience with electronic medical records (EMR) is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations and provide interventions for speech, language, communication, or swallowing difficulties.
• Develop treatment plans based on evaluations, physician orders, and goals.
• Revise care plans with the multidisciplinary BAYADA team as appropriate.
• Educate patients and families on rehabilitative care to promote health and independence.
• Document observations and interventions in the EMR using a touch pad tablet.
