Job summary

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for per diem home health visits in the Fairfax, VA area. The role focuses on evaluations, treatment planning, and ongoing care for adult and geriatric patients in private homes.

Qualified candidates will have VA licensure, ASHA CCC (or equivalent) and EMR familiarity. Home health experience is a plus, with comprehensive benefits and PPE support provided.

Qualifications

• Current VA licensure as Speech-Language Pathologist
• ASHA CCC or equivalent certification, or working toward
• Meets educational/transcript requirements or accumulating supervised experience
• Proficient with electronic medical records (EMR)

Responsibilities

• Visit clients in designated geographic territories
• Evaluate and diagnose speech, language, communication, or swallowing issues
• Develop and implement treatment plans based on evaluations and physician orders
• Revise care plans and collaborate with multidisciplinary BAYADA team
• Educate patients and families on rehabilitative care and activities
• Document observations and interventions on a tablet-based system
