Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification.

Responsibilities

• Evaluates the patient to determine rehab potential and creates an individualized plan of care.
• Sets initial short-term and long-term goals.
• Uses therapy equipment safely and maintains logs for equipment.
• Maintains equipment logs for outpatient clinic.
• Follows prescribed medical regimens and precautions.
