Job summary

Pathways Healthcare seeks a qualified Home Health Nurse (RN or LPN) in Massachusetts to perform patient assessments, administer medications, and develop individualized home-based care plans for rehabilitation and hospice patients.

The nurse will collaborate with physicians and interdisciplinary teams, provide caregiver support, supervise aides, and ensure compliance with healthcare regulations and BLS CPR standards. A valid MA license and reliable vehicle required.

Qualifications

• Active RN or LPN license in Massachusetts.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintains current BLS CPR certification.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Assess patients and families to determine home care needs.
• Develop and revise plans of care based on patient status.
• Collaborate with physicians and interdisciplinary teams on care plans.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

• Service Area: Beverly, Danvers, Lynn, Peabody, Middleton, Salem, Swampscott areas.

Responsibilities:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active RN or LPN license in the state where the position is based. LPNs are encouraged to apply.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
