Job summary

Pathways Healthcare in Boston, MA is seeking a dedicated Registered Nurse to join our in-home care team. You will assess patients, administer medications, and develop individualized plans of care to support recovery at home.

You will collaborate with physicians, nurse practitioners, and care teams, provide patient and caregiver education, supervise home health aides, and participate in discharge planning to ensure safe, dignified care in the home setting.

Qualifications

• Active nursing license in the state of Massachusetts.
• Preferably 2+ years of nursing experience focusing on geriatrics.
• Excellent bedside manners and communication skills.
• Knowledge of healthcare laws and regulations.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
