Job summary

Personal Touch Home Health Care Services Inc. seeks an experienced Registered Nurse to provide direct patient care across clinical settings, including acute and community environments. You will collaborate with healthcare teams to implement individualized care plans and ensure quality outcomes.

The role emphasizes patient safety, documentation accuracy, and active participation in orientation and precepting new staff. BLS is required; ACLS preferred.

Qualifications

Responsibilities

• Deliver safe, direct patient care to diverse age groups (Neonate to Geriatric).
• Assist with tests, procedures, medication administration (including IV) and emergency measures.
• Maintain accurate medical records and proper documentation.
• Participate in orientation, precepting new staff, and ongoing professional development.
• Ensure a safe, therapeutic environment and promote patient safety by following policies and engaging patients in their care.
• Answer inquiries, triage calls, and support discharge planning.

Role Overview

Provide direct patient care across various clinical specialties (e.g., PCU, ICU, ED) following nursing processes and policies. Serve as a role model to promote a positive work environment and ensure quality care. Collaborate with healthcare teams to develop and implement individualized care plans, including discharge planning and patient education.

Key Responsibilities

• Deliver safe, direct patient care to diverse age groups, including Neonate to Geriatric.
• Assist with tests, procedures, medication administration (including IV), and emergency measures.
• Maintain accurate medical records and ensure proper documentation.
• Participate in orientation, precepting new staff, and ongoing professional development.
• Ensure a safe, therapeutic environment and promote patient safety by following policies, reporting concerns, and engaging patients in their care.
• Answer inquiries, triage calls, and support discharge planning.

Qualifications & Skills

• Graduate of an accredited nursing program with at least 1 year of related experience.
• Current RN licensure and BLS certification; ACLS preferred.
• Additional certifications such as PALS, TNCC, or specialty credentials are advantageous.
• Ability to perform physically demanding tasks and work in environments requiring PPE due to hazards.
• Strong communication, teamwork, and patient safety focus.
