Job summary

Pleasant Acres Nursing & Rehabilitation Center is seeking a dedicated and compassionate RN Registered Nurse Per Diem to join our team, offering flexible shifts and per diem scheduling.

Provide high-quality, patient-centered care, administer medications, and coordinate with the interdisciplinary team to support residents' health and well-being.

Applicants must hold an active PA RN license in good standing and have experience in long-term care or geriatric settings.

Qualifications

• Active RN license in good standing in Pennsylvania.
• Previous experience in long-term care or geriatric settings is preferred.

Responsibilities

• Provide direct patient care including administering medications and wound care.
• Develop and implement individualized care plans with the interdisciplinary care team.
• Assess, monitor, and document patient progress and changes.
• Coordinate with physicians, therapists, and aides to ensure seamless patient care.
• Participate in quality improvement initiatives and staff education.
