Job summary

Hackensack Meridian Health is seeking a Registered Nurse to provide direct care as part of an interdisciplinary team in Perth Amboy, NJ. This per diem, day shift role emphasizes data collection, age- and disease-specific assessments, and collaborative care planning.

Ideal candidates are pursuing a BSN with a willingness to complete requirements within three years, have 6+ months of nursing experience or participate in the Nurse Residency Program, and hold an active NJ RN license with AHA BLS

Qualifications

• BSN in progress and/or willing to acquire within three years of hire.
• Six or more months of nursing experience or Nurse Residency participation.
• NJ State Professional Registered Nurse License.
• AHA BLS for HCP certification within 60 days of start.

Responsibilities

• Collect comprehensive data pertinent to the patient's health or situation.
• Perform age and disease specific assessment/data collection independently.
• Recognize clinical and diagnostic status changes in patient status.
• Identify variables in patient/family education needs based on age and disease.
• Develop a plan of care that prescribes strategies and interventions.
• Formulate age and disease specific patient/family centered goals/outcomes.
• Communicate, coordinate, and implement the plan of care.
• Provide ongoing communication with patient/family and healthcare team.
• Implement age and disease specific care in an organized manner.
• Perform continuous Point-of-Care electronic documentation.
