Job summary

As a Field Nurse (RN), you will perform patient assessments, manage medications, assist with ADLs, monitor vital signs, educate families, and document care in EHRs, while coordinating with the care team to ensure seamless,

Qualifications

• Current NJ RN license (unrestricted).
• ADN required; BSN preferred.
• At least 3 years of RN experience, with 3 years in home care or similar setting.
• CPR certification (BLS) required.

Responsibilities

• Conduct initial and ongoing patient assessments and develop care plans.
• Administer medications and ensure adherence to regimens.
• Provide ADL assistance with dignity and safety.
• Monitor vital signs and report changes to the care team.
• Educate patients and families on health and self-care.
• Maintain thorough EHR documentation and coordinate care.
