Job summary

Executive Home Care is seeking compassionate Field Nurses (RNs) to join our home care team in New Jersey. You will perform comprehensive patient assessments, monitor vital signs, manage medications, and support patients with daily activities in their own homes.

The role emphasizes independence, direct patient impact, and collaboration with a multidisciplinary team. A valid NJ RN license, ADN required with BSN preferred, and 3+ years of home care experience are expected; CPR/BLS and a valid

Qualifications

• RN license (New Jersey)
• ADN required; BSN preferred
• Minimum 3 years of RN clinical experience
• At least 3 years in home care, long-term care, or community setting
• Current CPR/BLS certification
• Valid driver’s license and reliable transportation

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Assist with medication administration and ensure adherence to regimens.
• Provide ADL support (bathing, dressing, feeding) and monitor comfort.
• Regularly monitor vital signs and report changes to the healthcare team.
• Educate patients and families on health and self-care techniques.
• Maintain accurate EHR documentation and coordinate care with the team.
