Job summary

Qualifications

• California RN license required at hire.
• 2 years of acute care clinical or comparable experience.
• Bachelor's degree in nursing preferred within 3 years of hire.
• CCM certification preferred at hire.
• 3 years of experience in community health nursing or case management.

Responsibilities

• Establish a continuum of care plan for a caseload of patients.
• Establish quality monitoring indicators, collection of data, analysis, documentation and reporting outcomes.
• Liaise with patients to facilitate transitions in care.
• Facilitate the transition for the patient from acute to the next level of care.
