Job summary

Qualifications

• California RN License required at hire.
• 2 years of acute care clinical or comparable experience.
• Bachelor's degree in nursing preferred within 3 years of hire date.
• CCM certification preferred.
• 3 years of experience in community health nursing or case management preferred.

Responsibilities

• Establish a continuum of care plan for a caseload.
• Establish quality monitoring indicators and data collection.
• Analyze and document outcomes and report results.
• Liaise with patients to facilitate transitions between care settings.
• Coordinate from acute to next level of care to ensure smooth progression.
