Job summary

Easterseals NJ is seeking a Residential Counselor to support group home residents with daily living activities and IRP-driven outcomes. This role is based in Essex County with a per diem schedule (minimum of 8 hours/month) and requires driving as needed.

You’ll educate residents on medications, document services, and coordinate care with the team while upholding the organization’s values and standards.

Qualifications

• Bachelor’s Degree in mental health, psychology, sociology, or social work from an accredited college or university.
• Minium of one year college plus related life or work experience equal to four years.
• High school diploma or GED plus four years of related work or life experience.

Responsibilities

• Support consumers in daily living activities and help achieve IRP outcomes.
• Educate consumers on medications, track logs, and conduct monthly audits.
• Coordinate services based on individual needs.
• Transport consumers using the company van as needed.
• Deliver high-quality care in line with IRPs.

What You’ll Do:

The Residential Counselor supports the group home residents with daily living activities and delivers high quality services that will help consumers achieve measurable outcomes in accordance with Individual Rehabilitation Plans (IRP).

Key responsibilities include:

• Support consumers in achieving personal care goals and addressing ongoing challenges.


• Educate consumers on medications, track logs, and conduct monthly audits.


• Coordinate services based on individual needs.


• Transports consumers using the company van as needed.


• Deliver high-quality care in line with Individual Rehabilitation Plans (IRP).


• Accurately document services within required timeframes using electronic health records.

Who You Are:

  • Bachelor’s Degree in mental health, psychology, sociology, or social work, human services or a related field from an accredited college or universityOR

• Skills:Strong verbal and written communication skills, with proficiency in Microsoft Office Suite, including Outlook, Excel, and Word.


• This role requires driving as an essential function. Candidates must have a valid driver's license and be subject to post-offer verification of driving eligibility consistent with company insurance requirements.
