Job summary

University Hospitals in Ohio seeks a qualified Physical Therapist to evaluate patients and develop individualized treatment plans within Rehabilitation Services at TriPoint Medical Center in Concord. The role is per diem/PRN with on-site work and involves direct patient care, education, and discharge planning.

You will collaborate with the care team, observe safety standards, complete documentation, and support department operations.

Qualifications

• Graduate of an accredited Physical Therapy program is required.
• Licensure as a Physical Therapist in Ohio and/or Multi-State Compact is required.
• BLS certification required within 90 days.

Responsibilities

• Performs evaluation(s) and develops an effective treatment plan appropriate to a patient's rehabilitation needs within the care continuum.
• Performs effective treatment techniques in a competent manner.
• Educates patient/family/caregiver in applicable health information, treatment and prevention.
• Initiates and participates in the discharge planning process.
• Performs other department activities such as documentation, billing, support of department operations.
