Job summary

Allegria Village is seeking a Per Diem Physical Therapist Assistant to join our Skilled Nursing Facility team in Dearborn. You will provide direct patient treatment under a licensed PT, assist with plans of care, and help residents achieve higher function and independence.

The role emphasizes accurate documentation, collaboration with an interdisciplinary team, and ongoing professional development in a smaller, in-house rehab program on a per diem basis.

Qualifications

• Graduate of an accredited PTA program.
• Current state license as a PTA in good standing.
• SNF experience preferred.
• Understanding of SNF standards and patient safety.

Responsibilities

• Provide direct patient treatment under supervision of licensed PT.
• Assist in implementing/modifying treatment plans for optimal outcomes.
• Monitor and document patient progress; report changes to PT.
• Instruct patients, families, and caregivers in exercises and safe mobility.
• Maintain accurate, timely documentation of care.
• Adhere to facility policies including safety and infection control.
• Communicate effectively with interdisciplinary team for continuity of care.
• Support a positive, team-oriented environment and patient satisfaction.
• Participate in ongoing professional development and trainings.
• Perform other related duties as assigned.
