Job summary

Center Management Group is hiring a Per diem Physical Therapist for Saturday shifts in Smithtown, NY. Must hold a valid NYS Physical Therapy license and have a degree in physical therapy. New graduates are welcome and will be considered for part-time weekend coverage.

The role offers competitive hourly rates ($50–$55 per hour) as a 1099 contractor with an excellent work environment and supportive team. Prior SNF experience is favored while maintaining high-quality PT services.

Qualifications

• Must have a Degree in physical therapy and a valid Physical Therapist NYS license.
• Strong in identifying patient needs, a team player, and efficient with time.

Responsibilities

• Perform evaluations and establish patient goals.
• Develop individualized treatment plans and discharge plans.
• Provide quality PT services with evidence-based interventions.
• Document progress toward objectives in the records system.
• Oversee PT Assistants and supervise on-site treatment interventions.
• Develop and implement new PT programs and techniques.
• Educate patients and families on care plans.
