Job summary

Smithtown Center for Rehabilitation & Nursing Care is seeking a Per Diem Physical Therapist to join their team. This role offers a competitive salary of $50–55 per hour and the opportunity to work in a positive environment while delivering quality patient care.

Qualifications

• Degree in Physical Therapy required.
• Valid NYS Physical Therapist license essential.
• At least 1 year of SNF experience preferred.

Responsibilities

• Conduct evaluations/screens and establish patient goals.
• Provide quality physical therapy services with evidence-based interventions.
• Document progress in the electronic medical record.
