Center Management Group is hiring a Per diem Physical Therapist for Saturday shifts in Smithtown, NY. Must hold a valid NYS Physical Therapy license and have a degree in physical therapy. New graduates are welcome and will be considered for part-time weekend coverage.

The role offers competitive hourly rates ($50–$55 per hour) as a 1099 contractor with an excellent work environment and supportive team. Prior SNF experience is favored while maintaining high-quality PT services.

• Perform evaluations and establish patient goals.
• Develop individualized treatment plans and discharge plans.

• Document progress toward objectives in the records system.
• Oversee PT Assistants and supervise on-site treatment interventions.
• Develop and implement new PT programs and techniques.
• Educate patients and families on care plans.

Physical Therapist Responsibilities:

• Must have 1 year SNF experience


• Performing evaluations/screens, establish patients goals, develops individualized treatment plans, developing treatment and discharge plans


• Provide quality physical therapy services encompassing personalized and evidence-based care and proven interventions


• Documenting progress toward identified objectives


• Overseeing Physical Therapy Assistant(s) and provides onsite supervision of treatment interventions and clinical documentation


• Developing and implementing new program/techniques to enhance physical therapy services.


• Conducting patient and family education


• Independently manage workflow which includes allotting time for treatment documentation of individual treatments and notes in the electronic records system

Must have a degree in Physical Therapy and must be licensed.
