Job summary

Qualifications

• Must have Bachelor's degree in physical therapy or equivalent.
• Licensed to practice physical therapy in Ohio.
• PT board certification (ABPTS) required.
• Driver's license required only for Home Care assignments.

Responsibilities

• Evaluates and treats patients; develops and implements plan of care.
• Plans and coordinates discharge with multidisciplinary team; instructs patient and caregiver/family.
• Documents evaluation, plan of care, progress toward goals, discharge plan, and related records.
• Supervises PTAs and rehab techs; participates in department quality improvement.
• Takes active roles in orienting new staff and serves as clinical instructor.
