• Provides direct care to patients and serves as a consistent care coordinator through the continuum of care, assessing the physical, psychological, spiritual and social needs of the patient.
• Directs patients to the next appropriate service and tracks their progress.
• In collaboration with Medical Director, assure that all hospital behavioral health patients receive appropriate evaluation, diagnosis, treatment, medical screening and medical/psychiatric evaluation whenever indicated, and that all medical/psychiatric care is appropriately documented in the medical record.
