Job summary

Center for Human Development (CHD) is seeking a Per diem Board Certified Psychiatric Mental Health Nurse Practitioner and Family Nurse Practitioner to work in CHD’s Crisis Stabilization and Outpatient Clinic in Chicopee, MA. This is an on-site position.

The role requires providing care to adults and adolescents, coordinating with a multidisciplinary team, and delivering clinical services in a structured clinic environment. This is an on-site position at our facilities.

Qualifications

• Masters or better.
• State certification as a Psychiatric Mental Health Nurse Practitioner or Clinical Nurse Specialist.
• Unrestricted APRN license.
• Mass Controlled Substance Registration.
• Mass Prescription Monitoring Program (PMP) registration.
• X-Waiver preferred.

Responsibilities

• Psychiatric evaluations and outpatient care for adults and adolescents.
• Coordinate with physicians, APRNs, psychologists, and social workers.
• Provide clinical consultation, in person or remotely, to other members of the clinical team.
• Document evaluations, assessments, diagnoses and treatments in the electronic health records.
• The ability to work fully on-site.
