Position Details:

Hartford HealthCare is seeking a Physician on a per diem basis to provide Skilled Nursing Facility (SNF) admissions and rounding coverage.

Role: Per Diem Physician, Geriatric patient population

Location(s): Stratford, CT

Setting: 100% Skilled Nursing Facility (SNF) – patient admissions and rounding

Schedule: Per Diem, flexible scheduling

Call: No call

• Supported by a team of APP’s that are in the facilities full time within a team of 18 Geriatric providers
• Strong support staff, nursing and operational team plus strong peer support to assist with any challenging situations that may arise
