Responsibilities

• Provide skilled therapy interventions including gait training, therapeutic exercise, and neuromuscular re-education.
• Collaborate with physicians, nursing, and rehab staff to support discharge planning.
• Document evaluations, progress, and discharge summaries in the EHR.
• Educate patients and caregivers on home exercise programs and safety.
