Job summary

Allegria Village in Dearborn seeks a Per Diem/Contingent Physical Therapist to join our Skilled Nursing Facility team. Provide direct patient care, perform evaluations, and develop individualized treatment plans to support residents' functional goals. The role requires state licensure in good standing and a PT degree.

SNF experience preferred. You will work with an interdisciplinary team, document thoroughly, and participate in continuing education.

Qualifications

• Graduate of an accredited PT program
• Current license in good standing with State Board of Physical Therapy Examiners
• SNF experience preferred
• Strong communication and teamwork skills

Responsibilities

• Provide direct patient care and perform functional evaluations to develop individualized treatment plans
• Design, implement, and adjust plans of care with family involvement
• Ensure therapy practices align with standards and facility policies
• Supervise and guide support staff to help patients meet functional goals
• Collaborate with interdisciplinary care team to promote positive outcomes
• Maintain compliance with quality improvement, safety, and infection control protocols
• Complete and maintain accurate documentation including evaluations, goals, plans, and prescriptions
• Participate in continuing education and professional development activities
• Foster positive relationships with patients, families, and colleagues
• Perform other related duties as assigned
