Allegria Village in Dearborn is seeking a Per Diem/Contingent Physical Therapist to join our Skilled Nursing Facility team. You will provide direct patient care, perform functional evaluations, and develop individualized treatment plans.

The role emphasizes collaboration with the interdisciplinary team, adherence to standards and policies, accurate documentation, and ongoing professional development to deliver high-quality care to residents.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current license in good standing with the State Board of Physical Therapy Examiners.
• SNF experience preferred.
• Knowledge of applicable care standards and protocols.
• Strong communication and empathy with residents and families.

Responsibilities

• Provide direct patient care and perform functional evaluations to guide treatment plans.
• Design and adjust plans of care to achieve optimal outcomes with family involvement when appropriate.
• Ensure therapy practices align with standards of care and facility policies.
• Supervise support staff to help patients meet functional goals.
• Collaborate with interdisciplinary care team to promote positive outcomes.
• Maintain compliance with facility policies including safety and infection control.
• Document evaluations, goals, plans, and physician prescriptions accurately.
• Engage in continuing education to stay current with best practices.
• Foster positive relationships with patients, families, and colleagues.
• Perform other related duties as assigned.
