Flagship Rehabilitation, part of Enhance Therapies, is seeking a licensed Physical Therapist for a per diem position in a SNF/long-term care setting. Compensation is $60.00–$65.00 per hour. Must be a licensed PT to apply.

The role involves evaluating mobility and functional impairments, creating personalized care plans, and collaborating with the care team to achieve quality outcomes while maintaining compliant documentation and up-to-date physician orders.

Qualifications

• Must hold a license as Physical Therapist in the state of practice.
• Graduated from an APTA-accredited PT program (BS, MS, or DPT).
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments
• Create and implement personalized care plans
• Collaborate with team members to ensure quality outcomes
• Maintain documentation to meetregulatory requirements
