Job summary

Polaris Healthcare and Rehabilitation Center is seeking a caring Physical Therapist to join our rehab team in Milford, Delaware. Qualifications

• Experience providing physical therapy in a skilled nursing facility.
• Graduated from an accredited physical therapy program.
• Current Physical Therapy license for the state of Delaware; CPR certificate required.
• Good organizational, time management, and communication skills.

Responsibilities

• Develop individualized plans of care to facilitate optimal performance using standardized tests and measures.
• Identify needs and make referrals to other disciplines to address the comprehensive needs of patients.
• Monitor patients' progress and adjust treatments accordingly.
• Provide supervision for Physical Therapy Assistants, students, and support personnel in accordance with state license requirements and professional standards.
• Perform other duties as assigned to provide quality care to residents.
