Job summary

JAG Physical Therapy is seeking a Per Diem Physical Therapist to fill in as needed across our outpatient clinics. The PT will assess patients, design creative treatment plans, and document care while maintaining confidentiality in a high-energy environment.

Responsibilities include evaluations, daily notes, goal setting, and coordination with the care team. A state license is required and CEU requirements in NY/NJ must be completed. Prior EMR experience is preferred.

Qualifications

• Bachelor's or Master's degree required; Doctorate degree preferred.
• Licensed Physical Therapist in the state.
• Complete CEU requirements in NY/NJ.
• Experience with EMR software preferred.

Responsibilities

• Perform initial evaluations and document plans of care.
• Record daily treatment and progress notes; support billing.
• Develop and implement treatment plans with goals and prognosis.
• Provide musculoskeletal interventions and therapeutic exercises.
• Educate patients and families; ensure safety and confidentiality.
