Qualifications

• Current state licensure or eligibility for state licensure as a Physical Therapist.
• Certified BLS or completion in the first 90 days of employment.
• Current State Driver's License in good standing.
• One or more years of clinical experience with neuro rehabilitation or neuro transitional experience.

Responsibilities

• Evaluates all assigned patients and develops, implements and modifies the plan of care in collaboration with the transdisciplinary team.
• Delivers therapy across multiple environments and educates patients, families, and staff.
• Participates in discharge planning to facilitate safe discharge home.
• Completes medical record documentation and charge entry in compliance with policies.
• Actively participates in student programs and provides professional development.
