SSM Health Rehabilitation Hospital - Neuro Transitional Center in O'Fallon, MO is seeking a Physical Therapist for a per diem role. You will evaluate patients, implement and adjust plans of care with a transdisciplinary team, and deliver therapy across various settings to promote independence after brain or spinal injuries.

You will participate in discharge planning, educate patients and families, and document clinical progress while adhering to regulatory and facility guidelines.

Formación

• State licensure or eligibility for licensure as a Physical Therapist.
• BLS certification or completion within 90 days of employment.
• Valid driver's license required.

Responsabilidades

• Evaluates patients and develops/modifies the plan of care with the transdisciplinary team.
• Delivers therapy across multiple environments to maximize patient outcomes.
• Participates in discharge planning and family education.
• Documents medical records and ensures proper billing entries.
• Supports Neuro Rehab Coaches and trains family in home/community settings.
