Job summary

NorthBay Health in Fairfield, CA is seeking a Physical Therapist (per diem, days) to work under the supervision of the Rehabilitation Supervisor and Rehabilitation Services Manager. The therapist evaluates patients with physician referrals, develops treatment plans and goals, administers modalities, and plans for discharge using appropriate techniques.

Qualifications

• Minimum bachelor’s degree in Physical Therapy or equivalent.
• Current California license in Physical Therapy is required.
• One year of experience in acute hospital or outpatient clinic preferred, however, will consider new graduates.

Responsibilities

• Provides physical therapy services in inpatient and outpatient settings during assigned shift during department operating hours.
• Evaluates patients, develops treatment plans and goals, administers modalities, and plans for discharge.
• Acts as Rehab case manager and maintains accurate patient records.
• Re-evaluates patient progress and communicates with physicians and payers.
• Instructs patients and families and supervises the Physical Therapist Assistant.

Education

Bachelor's degree in Physical Therapy

Current California license in Physical Therapy

AHA/BLS certification or equivalent

One year of experience in acute hospital or outpatient clinic (preferred)
