Job summary

Lehigh Valley Health Network in the Lehigh Valley is seeking a licensed Physical Therapist to evaluate, plan, direct, and administer rehabilitation programs at the Health Center at Macungie.

You will develop and update individualized plans of care, apply evidence-based therapies, and monitor patient progress across shifts, with per diem travel to various home care locations as needed. Collaboration with physicians and the rehab team is essential to restore function and independence.

Qualifications

• Bachelor’s Degree Physical Therapy.
• Ability to work at various locations per diem.
• Knowledge of best practices in physical therapy.
• Sound clinical reasoning in evaluating data and determining diagnoses.
• Able to provide safe, compassionate, and competent physical therapy.
• Effective communication with patients, families and co-workers.
• Willingness to travel to Home Care assignments.
• American Heart Association Basic Life Support – PA Upon Hire.
• PT – Licensed Physical Therapist – PA Upon Hire.

Responsibilities

• Establish and implement PT plans of care to restore function and independence.
• Apply evidence-based therapies and modalities and functional mobility training.
• Provide PT treatment aligned with patient assessment.
• Reassess as needed and modify plans accordingly.
• Communicate progress to supervisors, physicians and the care team.
• Ensure goals/timelines are measurable within care plans.
• Supervise as required by regulatory standards.
• Deliver cost-effective care and complete required documentation.
