Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Ability to work at various locations on a per diem basis.
• Knowledge of best practices in physical therapy.
• Demonstrates sound clinical reasoning in evaluating data to identify treatment diagnoses.
• Must provide safe, compassionate, competent physical therapy.
• Ability to communicate with patients, families and co-workers in a friendly, supportive manner.
• Ability to travel to patient care assignments - Home Care.

Responsibilities

• Establishes and implements physical therapy plans of care to restore function and independence.
• Uses evidence-based therapy techniques such as therapeutic exercises and manual therapies.
• Performs reassessments and updates plans of care as needed.
• Communicates patient progress and limitations to supervisor and care team.
• Provides age/population appropriate therapy with measurable goals and timelines.
• Ensures documentation aligns with policies and regulatory requirements.
