Cayuga Health is seeking a per diem Physical Therapist to provide patient-centered care at Cayuga Medical Center in Ithaca, NY. Responsibilities include clinical decision-making, patient education, and interdisciplinary collaboration to ensure safe, effective therapy and progress toward goals.

The role requires completion of an accredited PT program, NYS PT license, and CPR certification, with yearly safety trainings.

Formación

• Education: Successful completion of accredited physical therapy program; may be Bachelor's, Master's, or Doctorate level program.
• Licensure: Current NYS Physical Therapy license
• Certification(s): Current CPR Certification
• Continual Training: Must complete yearly mandatory safety trainings and e-learning courses

Responsabilidades

• Clinical Decision-Making: Completes comprehensive chart review to determine PT appropriateness and plans of care.
• Communication: Educates patient/family and communicates with the interdisciplinary team.
• Safety: Identifies contraindications and ensures safe patient handling and environment.
• Patient Management: Progresses interventions and documents progress clearly.
• Discharge Planning: Determines discharge needs and follow-up requirements.
