Job summary

UW Health Beaver Dam Clinic seeks a part-time Physical Therapist to join an on-site team in Beaver Dam, WI. You will collaborate with an interdisciplinary team to develop evidence-based treatment plans and maximize patient outcomes.

Required: Bachelor's in Physical Therapy, Wisconsin licensure upon hire, BLS within 3 months; 2 years' experience preferred. UW Health offers flexible schedules, relocation assistance, and opportunities for professional growth.

Qualifications

Bachelor's Degree in Physical Therapy required.
2 years of prior relevant experience preferred.
Wisconsin licensure Upon Hire required.
Basic Life Support (BLS) certification within 3 months required.

Responsibilities

Collaborate with the interdisciplinary team to create comprehensive evidence-based treatment plans to maximize functional outcomes and optimize discharge.
Provide patient and family centered care in a fast paced, dynamic environment at a nationally recognized academic medical center.
Demonstrate a high level of clinical excellence, autonomy, adaptability, and engagement.
Commit to on-going learning to advance their clinical knowledge, skills, and abilities through peer mentorship, clinical education, and other learning opportunities.
Take an active role in program development and quality improvement.
