Job summary

Pathways Healthcare is seeking a licensed Physical Therapist to deliver in-home physical therapy services under a physician's plan of care. This per diem role covers our home health patients in York, ME and surrounding areas, with focus on rehabilitation and return to daily activities.

Responsibilities include assessments, treatment administration, caregiver training, discharge planning, and collaboration with a care team.

Qualifications

• Possess a degree from an accredited Physical Therapy program.
• Hold a valid Physical Therapist license in Massachusetts.
• Minimum of two years' experience; home health preferred.

Responsibilities

• Deliver PT services per physician's plan of care and state PT Act.
• Assess therapeutic, rehabilitative, and functional status including home environment.
• Perform strength, mobility, gait, and ROM assessments to identify rehab potential.
• Administer PT treatments and educate on use and care of therapeutic devices.
• Report patient response to treatment or changes in condition to the physician.
• Initiate and manage PT programs, collaborating with the care team and family.

Education

Valid Physical Therapist license in Massachusetts
