Job summary

One on One Physical Therapy LLC is seeking a Per Diem Physical Therapist in New York to provide orthopedic therapy on an as-needed basis. You will evaluate patients, develop treatment plans, document care, and collaborate with the care team to support recovery and functional goals.

Responsibilities include initial evaluations, progress notes, discharge planning, and patient education. NY licensure, CEU compliance, and EMR experience are required or preferred.

Qualifications

• Bachelor's or master's degree required; doctorate preferred.
• Licensed Physical Therapist in the state practicing in NY.
• Completed state CEU requirements (NY/NJ).
• Experience with EMR software is preferred.
• Excellent oral and written communication.
• Strong clinical decision-making and work ethic.

Responsibilities

• Collect and record patient care data per guidelines and payer requirements.
• Perform initial evaluation, daily notes, progress reports, and discharge planning.
• Document daily billing and maintain productivity.
• Prepare evaluations and gather patient history with diagnoses knowledge.
• Establish treatment plans and modify as needed; collaborate with the team.
• Provide education and functional training to patients and support system.
