Job summary

Prestige Care Center of Plattsmouth is seeking a flexible Per Diem Physical Therapist (PT) to join our rehabilitation team at 25–30 hours per week. You will evaluate residents, design individualized treatment plans and deliver skilled PT services in a skilled nursing environment.

The role requires a Nebraska PT license, a DPT, CPR certification (preferred), and a driver’s license. Collaboration with doctors, nurses and other therapists is essential to improve mobility, independence and quality

Qualifications

• Current, valid Physical Therapist license in Nebraska.
• Graduate of an accredited Physical Therapy program (DPT).
• Meets all educational and licensing requirements for state practice.
• CPR certification (preferred).
• Valid driver's license.
• Knowledge of skilled nursing regulations, Medicare documentation requirements, and long-term care best practices.
• Strong communication, clinical reasoning and documentation skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans to restore function and mobility.
• Provide skilled PT services per physician orders and facility policies.
• Assess gait, balance, strength and functional mobility; set measurable goals.
• Implement evidence-based interventions to improve outcomes and reduce fall risk.
• Complete all clinical documentation, billing and coding accurately.
• Collaborate with physicians, nursing staff and other therapists to coordinate care.
• Participate in care conferences, discharge planning and interdisciplinary meetings.
• Educate residents and families on exercises, mobility techniques and home programs.
