Job summary

Berkshire Rehabilitation & Skilled Care Center in Sandisfield, MA is seeking a Physical Therapist to join our part-time and per diem team. You will conduct patient assessments, develop care plans, provide direct treatment, and support caregivers within a 57-bed all-male skilled nursing facility.

The role requires collaboration with the clinical team, supervision of staff and students, and adherence to Medicare reimbursement guidelines. Up to $55/hour with per diem opportunities.

Qualifications

• Bachelor’s/Master’s/Doctoral degree in Physical Therapy.
• Appropriate licensing requirements by individual states must be maintained.
• Strong oral and written communication skills.

Responsibilities

• Screen patients/residents and evaluate needs.
• Develop and implement care plans and provide direct patient treatment.
• Teach and supervise caregivers, staff, students, and other therapists.
• Document care activities and ensure timely charting.
• Participate in program development, census-building, and quality improvement.
