Qualifications

• DPT required; only if license obtained on or after January 2016
• CPR or BLS certification required
• Active state licensure in practice location

Responsibilities

• Conduct and plan treatment segments to restore physical and functional abilities
• Co-lead interdisciplinary patient groups and coordinate care
• Complete evaluations and establish individualized care plans
• Document clinical activities accurately within deadlines
• Supervise assigned assistants
