Evaluates, plans, directs and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries and maintains patient files. Counsels patients and/or family members regarding Rehabilitation diagnosis, plan of care and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to Physical Therapy and adheres to same.
For Wayne employees only: Quality Indicator (QI) certification required to be taken following employment training and then certification must be taken through the rehab program (UDSMR).
For EIP employees only: a valid driver’s license with no pending violations and an acceptable driving record as stipulated by SJH policy and, if applicable, the insurance carrier, for those required to drive a hospital vehicle.
Effective January 2019, St. Joseph’s Health requires all employee certifications for Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) to be granted through the American Heart Association.

St. Joseph’s Health provides a salary range to comply with New Jersey law. The rate of pay for each position will be determined based on experience, qualifications, and skills. The salary range does not include incentives or other forms of compensation.
